This session will provide leaders of hospitals, health systems and physician groups a comprehensive understanding of the dual eligible population and the financial imperatives, resource use, and care coordination requirements this population creates for their organizations. Industry leaders will gain insight on how to restructure their care management processes, build appropriate infrastructure, and align incentives in order to effectively manage dual eligible patients across the continuum of care. Topics include:
  • The size and scope of the dual eligible population, including a typical profile of a dual eligible beneficiary and how to identify and target dual eligible sub-populations
  • The economic impact dual eligibles have on the current healthcare system, including a detailed look at dual eligible spend for Medicare and Medicaid and insights into acute and post-acute utilization
  • The challenges associated with serving the dual eligible population, coordination of care difficulties, and the wide range of conditions and health needs
  • Care delivery transformation and reimbursement models tailored specifically for the dual eligible population with a discussion of current models in use
  • Preparing your organization for the impact of the dual eligible population and specific actions and capabilities that should be developed

As one of the nation's largest providers of managed Medicare, Davita HealthCare Partners is poised to be a health delivery system of choice for the huge wave of new dual eligible patients being auto-enrolled into managed care. In preparation, the organization underwent a concentrated effort to successfully develop and implement a care management structure redesign that focused on home and community based services, long term care prevention, and special collaborative care centers focused on the chronically mentally ill.

This session will demonstrate how to ensure success in treating the most challenging patient population enrolled in managed care. It will teach attendees about successfully collaborating with health plans, government, the provider community, and most importantly patients and their families. Topics of discussion will focus on best practices in marketing, patient experience, intake and empowerment of patients and families, integrating medical and psychosocial treatment with long term care coordination, and breaking the cost curve while improving care quality and clinical and fiscal outcomes.
